First Visit

When you come for your appointment please remember to obtain and bring the following

• Referral letter from GP, family physician or other doctor
• Patient Information and Registration Form (If you have not received this form please call 4252 7333)
• Medicare card, DVA card, Pension Card
• Reports, X-rays, MRIs, CT scans etc. and any other relevant information
